Crush fatality in Ontario

A man working from a boom lift died after he was crushed between the platform's guardrail and an overhead beam during the steel erecting stage of a new industrial building in Vaughan, near Toronto, Canada.

The man was apparently working from a JLG telescopic boom lift on Monday when the entrapment incident occurred. The local ministry of labour issued a statement saying: "A worker sustained fatal injuries after being pinned between the guardrail of an elevated work platform and the structural steel of a building."
