Kanazawa University research: Potential drug treatment for particular type of lung-cancer

KANAZAWA, Japan, Sept. 16, 2020 /PRNewswire/ -- Researchers at Kanazawa University report in Nature Communications the mechanism making some lung-cancer patients resistant to the drug osimertinib. In addition, they suggest a combined drug treatment resolving osimertinib resistance in the case of cancer cells expressing low amounts of AXL, a protein belonging to the class of receptor tyrosine kinases.

The effectiveness of cancer treatment is often hampered by cancer cells being heterogeneous.  This is the case for EGFR-mutated lung cancer: drugs based on biomolecules of a type known as tyrosine kinase inhibitor (TKI) have been used to treat the disease, but with various levels of efficacy. (EGFR stands for "epidermal growth factor receptor", a protein playing an important role in signaling processes from the extracellular environment to a cell.) Sometimes, tumor cells are simply resistant to the drug.  Now, Seiji Yano from Kanazawa University and colleagues have investigated the efficacy of the TKI osimertinib for treating EGFR-mutated lung cancer, and how it relates to the expression in tumor cells of a particular protein called AXL.  They found that both AXL-high and -low expressing tumor cells showed tolerance (acquired resistance) to osimertinib, but that the mechanisms involved are different for the two situations.  Moreover, the researchers suggest a way to enhance the success of osimertinib treatment for the case of AXL-low expressing tumors.

First, the scientists compared the susceptibility to osimertinib in both AXL-high and -low expressing tumor cells in in vitro experiments.  They observed that osimertinib inhibited the viability of the cancer cells in both cases, but that the sensitivity to the drug was higher for AXL-low expressing EGFR-mutated lung cancer cells. They also noticed that a small number of tumor cells survived the procedure — an indication of osimertinib tolerance. These findings were consistent with results from the clinical study of the drug performed earlier on 29 patients with EGFR-mutated non-small cell lung cancer.

Through experiments aiming to understand the mechanism behind osimertinib tolerance, Yano and colleagues discovered that phosphorylation of IGF-1R was increased in AXL-low-expressing tumor cell lines, but not in AXL-high expressing tumors. (IGF-1R stands for 'insulin-like growth factor 1 receptor'; it is a protein located on the surface of human cells.  Phosphorylation is the chemical process of adding a phosphoryl group.)  The researchers then found that phosphorylated IGF-1R supported the survival of AXL-low expressing tumors after exposure to osimertinib.

The scientists then tested whether the observed osimertinib resistance could be resolved by administering linsitinib, a substance known to inhibit the phosphorylation of IGF-1R.  Encouraged by the positive outcome of the experiment, Yano and colleagues went further and evaluated the combination of osimertinib and linsitinib.  Their conclusion was that the transient combination of linsitinib with continuous osimertinib treatment could cure or at least dramatically delay tumor recurrence in AXL-low-expressing EGFR-mutated lung cancer.  More investigating needs to be done, though.  Quoting the researchers: "… the safety and efficacy of the transient combination of IGF-1R inhibitor and osimertinib should be evaluated in the clinical trials."

Background

Tyrosine kinase inhibitors

A tyrosine kinase inhibitor is a drug inhibiting (that is, preventing or reducing the activity of) a specific tyrosine kinase. A tyrosine kinase is a protein (enzyme) involved in the activation of other proteins by signaling cascades. The activation happens by the addition of a phosphate group to the protein (phosphorylation); it is this step that a tyrosine kinase inhibitor inhibits.  Tyrosine kinase inhibitors are used as anticancer drugs.  One such drug is osimertinib, used to treat EGFR-mutated lung cancer.

AXL

AXL is a receptor tyrosine kinase — a tyrosine kinase consisting of an extracellular part, a transmembrane part ('sitting' within a cell membrane) and an intracellular part.  AXL regulates various important cellular processes, including proliferation, survival and motility.  

In recent years, it has become clear that AXL is a key facilitator of drug tolerance by cancer cells.  Seiji Yano from Kanazawa University and colleagues have found that this is also the case for EGFR-mutated lung cancer.  While a high expression of AXL correlates with resistance to osimertinib, such tolerance also occurs in AXL-low-expressing cancer cells.  Yano and colleagues have now found that for the latter case, phosphorylation of IGF-1R (insulin-like growth factor 1 receptor) is responsible for the resistance to osimertinib.

Walmart Inc's Flipkart said on Tuesday it would create 70,000 new jobs and employ many more as delivery partners and in other roles as it prepares for a surge in online shopping during the busy Indian festive season.

Flipkart, Amazon.com Inc's Indian unit and Reliance Industries Ltd's fledgling e-commerce business are vying for a share of the booming online retail market, which received a big boost from the COVID-19 pandemic as more Indians used their smartphones to shop for groceries and other items.

Flipkart’s “Big Billion Days”, styled on the lines of Amazon’s Prime Day, rakes in its biggest sales for the year. The four- or five-day long sale usually starts around October to tap India’s festive season, which ends with Diwali.

The company said it would also sign up more than 50,000 kiranas, or small grocery stores, for last-mile delivery.

“While creating direct job opportunities across Flipkart’s supply chain, which include delivery executives, pickers, packers and sorters, there will also be additional indirect jobs created at Flipkart’s seller partner locations and (local corner stores),” the company said in an emailed statement.

Earlier this month, the e-commerce company launched an online wholesale service, Flipkart Wholesale, for mom-and-pop stores and other small businesses.

Biden assembles legal team ahead of divisive 2020 election

Democrat Joe Biden is assembling a team of top lawyers in anticipation of court challenges to the election process that could ultimately determine who wins the race for the White House.
Biden's presidential campaign says the legal war room will work to ensure that elections are properly administered and votes correctly counted. It will also seek to combat voter suppression at the polls, identify foreign interference and misinformation, and educate voters on the different methods available for casting ballots.

The effort, which the Biden campaign described as the largest election protection program in presidential campaign history, reflects the extent of the preparation underway for an already divisive presidential contest in November that could produce significant, perhaps even decisive, court cases over voter access and the legitimacy of mail ballots.

Democrats and Republicans are locked in legal fights on election rules that could help shape the outcome of the vote, and President Donald Trump's campaign has its own attorneys handling cases on a variety of issues.

Trump in recent months has sought to preemptively cast doubt on the election, warning that the expected surge in mail ballots because of the coronavirus pandemic will lead to massive fraud and could open the door to foreign countries to print their own fraudulent ballots.

Notwithstanding Donald Trump and his Republican allies' hollow threats and constant misinformation, election officials around the country are working tirelessly to hold a free and fair and election, and we have an extraordinary national team in place to ensure that every eligible voter is able to exercise their right to vote and have their vote counted," Bob Bauer, a senior adviser to the Biden campaign and former White House counsel to President Barack Obama, said in a statement.

Bauer, who served as general counsel to the Obama campaigns of 2008 and 2012, will work with campaign general counsel Dana Remus on voter protection an issue that thousands of Democratic lawyers around the country are also engaged in, according to the Biden campaign.

The campaign is also creating a special national litigation team involving hundreds of lawyers that will include as leaders Walter Dellinger, a solicitor general in the Clinton administration, and Donald Verrilli, a solicitor general under Obama.

Democratic lawyer Marc Elias and a team of lawyers from his firm, Perkins Coie, will focus on protecting voter access and ensuring a fair and accurate vote count.

Former Attorney General Eric Holder will also play an outreach role on the question of voting rights, according to the campaign.

We can and will be able to hold a free and fair election this November and we're putting in place an unprecedented voter protection effort with thousands of lawyers and volunteers around the country to ensure that voting goes smoothly, Remus said in a statement.

The Trump campaign issued a statement from deputy manager Justin Clark saying that as "Democrats continue their push to weaken reasonable rules preventing fraud like voter ID and signature matching President Trump and his campaign will continue to protect the integrity of the vote.

Our team will continue to fight every day in the courtroom and on the ground to make sure that every eligible voter has the right to vote and that their vote is counted once, Clark said.

Already regarded as the world's richest and most successful Twenty20 cricket team, the Mumbai Indians will now attempt to underline their status by claiming their first back-to-back titles in the Indian Premier League. Led by senior Indian batsman Rohit Sharma and owned by Nita Ambani -- wife of India's richest man, Reliance Industries billionaire Mukesh Ambani -- Mumbai have won an unsurpassed four IPL titles. So far they have been unable to match the Chennai Super Kings' feat of 2010 and 2011 by winning the world's most lucrative T20 league twice in a row.

But Mumbai, who open the coronavirus-delayed IPL season against the Super Kings in Abu Dhabi on Saturday, head into the tournament as the bookies' favourites for a fifth trophy.

Mumbai's financial clout is formidable. Ambani, who bought the franchise for more than USD 100 million in 2008, has been rated by Forbes as the world's second richest sports team owner behind former Microsoft chief executive Steve Ballmer, who owns the Los Angeles Clippers.

According to an Economic Times estimate, the Indians are worth USD 115 million, the highest among the eight IPL franchises.

The team has an impeccable playing pedigree too. Their founding captain was the now retired Sachin Tendulkar, the record-breaking batsman who is worshipped by Indian fans. Former Australia captain Ricky Ponting is another ex-skipper.

- 'Absolute greats' -

But it was only when Sharma was appointed captain in 2013, after Ponting stepped down due to loss of form, that the team became a force to be reckoned with.

The 33-year-old star batsman has led by example, scoring 3,728 runs since joining Mumbai in 2011 and captaining the team to IPL trophies in 2013, 2015, 2017 and 2019.

Sri Lankan legend Mahela Jayawardene is the coach, while West Indian big-hitter Kieron Pollard and India quick Jasprit Bumrah add power to the team.

Australia paceman Nathan Coulter-Nile, part of the Indians' first title win in 2013, returns this season after being snapped up for USD 1.12 million in the December auctions.

"It's good to come back to the most successful T20 franchise of the world," said Coulter-Nile.

"Obviously the team I was involved with then was filled with absolute greats. Ricky Ponting, Sachin Tendulkar, Harbhajan Singh, Rohit Sharma.

"And to come back to see this team now developing those sort of greats again. Rohit Sharma is a great in himself. Bumrah, best death bowler in the world."

However, the loss of pace ace Lasith Malinga earlier this month was a big blow, after the veteran Sri Lankan opted out of the tournament to be with his family.

But despite the loss of Malinga, the IPL's highest wicket-taker with 170 scalps in 122 matches, Mumbai's fans still expect their team to come home with the trophy.
