Serial paedophile Michael Guider's brother believes it is only a matter of time before he reoffends now that he has been released from prison.
The 68-year-old, who killed Sydney schoolgirl Samantha Knight in 1986, was released from jail on Thursday with a massive hernia protruding from his gut underneath his loose grey shirt and black pants.
A source said Guider told him he had been suffering from a painful, ever-expanding hernia for about 12 months but refused to have it treated because he was too scared to visit a prison hospital.
It is unclear yet what treatment he will seek now that he has been released from prison, but his brother Tim Guider told 60 Minutes he had no doubt the 'evil' Guider would prey on another victim now that he is a free man.
'He looks like the personification of evil and he is,' he said.
'He hasn't changed. If anything, he's worse. What's going to happen if he does kill another child? Whose child is it going to be?'

Despite his brother's concerns, a court last week ruled Guider was no longer enough of a threat to the community to remain behind bars.
He had a series of 56 'extreme' restrictions imposed on him in exchange for his freedom that limits his capability to move and travel at will.
Guider appeared to be uncomfortable as he made his way from prison, which is likely due to the bulging hernia.
A hernia is the protrusion of organs such as intestines through a split in a weakened section of the abdominal wall.
If left untreated, as Guider's has been, the split in the wall widens and more tissue or organs are pushed through the gap to form an external sac.
'I've talked to him about it,' the source said. 'It's a hernia. Basically that's his lower intestines hanging outside his lower abdomen.
'It's not life-threatening but it's painful.

'He hasn't wanted to have it treated because he's afraid of going into the hospital.
'He's afraid of going in there because there's maximum security prisoners in there and they'll likely attack him.
'So he's going to wait until he gets out, he said, so he can go to a normal hospital and have it done properly.'
Guider swayed as he walked free from the Long Bay prison complex after serving 23 years for the manslaughter of nine-year-old Samantha and other sex crimes against children.
The hernia was obviously causing him discomfort as he left the jail carrying two black garbage bags filled with his belongings.
Guider had resisted being moved from the Metropolitan Special Programs Centre to Long Bay's prison hospital as the hernia continued to grow.
'I said to him, 'They'd put you in a separate room on your own that's locked',' the source said.
'He said, 'But there'd still be nurses or doctors coming and going and the other prisoners might get the chance to attack me.' That was his reasoning.'

Passers-by hurled insults at Guider after he left the prison, while a motorcyclist screamed 'you filthy animal' and a driver yelled 'you filthy piece of s***'.
Sydney grandmother Michele Koroi, who lives near the jail, yelled: 'You're a dog, and you'll never change, you should stay locked up.'
Guider pleaded guilty in 2002 to the manslaughter of Samantha Knight on August 9, 1986. She had disappeared from her Bondi home.
He has never revealed where her body was buried.
The paedophile was already serving a 16-year sentence for 77 sexual offenses committed against 13 children between 1980 and 1996 when he was jailed over Samantha's death.
A bearded and bedraggled Guider maintained a downward gaze as he walked from jail towards a waiting car, escorted by prison staff.
He made no comment to reporters, was heckled by two women, was yelled at by onlookers and ignored questions about where Samantha's body was buried.
Guider will live temporarily in the Nunyara Community Offender Support Program Centre temporarily, which is within the Long Bay prison complex.
The facility is one block from local childcare centre called the Blue Gum Cottage.

He will be subject to a five-year extended supervision order with 56 'extremely restrictive' conditions, including that he be electronically monitored 24/7 with an ankle bracelet.
Guider will not be allowed near places where children gather including schools, pools and cinemas, his internet usage will be monitored, and he won't be allowed to change his personal appearance without permission.
NSW Attorney-General Mark Speakman had sought legal advice about challenging Tuesday's Supreme Court decision which rejected his application to keep Guider behind bars for another year.
But late on Wednesday Mr Speakman concluded any appeal would fail.
Mr Speakman on Thursday described Guider as 'one of the most despicable individuals to come before our justice system'.
'I'm sorry we have failed to get a continuing detention order, but I can guarantee the community's safety is the number one priority,' he said.
'This offender will not enjoy freedom the same way (as) ordinary citizens.
'He is a high risk of reoffending which is why we have sought this order to watch him like a hawk.'
Samantha's mother, Tess Knight, told reporters earlier this week she was angry and 'shaking inside' at the prospect of his release.
