A woman accused of punching and kicking a mother-to-be during a random attack has faced court for the first time.

Olivier Mahe, 28, was dressed in prison greens when she appeared in Sydney's Central Local Court by video link from Silverwater Women's Correctional Centre's mental health unit on Tuesday, where she's currently being held awaiting further assessment.

The western Sydney woman was charged with assault occasioning actual bodily harm after a heavily pregnant woman was allegedly attacked on her way to work last month.

Police allege the victim Alexandra Cojanu, 26, was repeatedly punched in the head, had her hair pulled and rammed into a pole before she fell into the gutter in Sydney's Surry Hills on May 28.

The young woman was 30 weeks pregnant at the time.

The alleged attack was captured on CCTV, which shows Ms Cojanu clutching her stomach and curled into a ball to protect her unborn child before Good Samaritan passer-by Martin Barrett rushed to her rescue.

An off-duty probationary police officer who had been walking past also rushed to Ms Cojanu's rescue and pulled her alleged attacker away.

Ms Cojanu was taken to hospital suffering bruising and swelling to her face, where she was later discharged.

Her unborn baby was unharmed in the alleged attack, which Detective Chief Inspector Adam Johnson described at the time as totally unprovoked, unwarranted, and unnecessary.

Mahe's lawyer Danielle Roth asked for a two-week adjournment for a Justice Health report to be prepared during Tuesday's brief court hearing.

'She is currently in the mental health unit,' she told the court.

Magistrate Robert Williams formally refused bail and listed the matter for June 25, where he ordered a plea be entered on that date.
