Pictures of the restaurant kitchen show mouldy chopping boards and heavily rusted saucepans being used to prepare food.
A spokesperson for the City of Fremantle told The West Australian that inspectors found a raft of breaches, including an open drain in the floor 'covered in mould and slime'.
'Waste water appeared to spill from the wash sink, drains and bucket trap and flow around into the cool room, posing a further risk of cross contamination.' the spokesperson said.
They also said a chest freezer was observed to be 'rusty, mouldy and full of old food scraps' and the cool room door 'covered in a build-up of dirt, mould and grease'.

Authorities found there was no chemical sanitiser to clean surfaces and the freezer was found to be rusty
Among other disturbing discoveries in the restaurant kitchen were that there was no chemical sanitiser to clean food surfaces, the dishwasher was not working, and dishwashing racks were mouldy.
There was also no hot water, hand soap, or hand towels for staff to use.
The business owner, Nilish Dokhe, was not present at the inspection but told the publication the kitchen was cleaned to the proper standard the next day.
Other issues, such as cracked ceiling tiles were fixed in the following weeks.
The breaches amounted to a fine of $25,000 for the restaurant.
It is the fourth business to be named as breaching the Food Act by the City of Freemantle this year.
Krusty Kob bakery was fined $10,000 for selling a mouldy birthday cake.
An IGA supermarket had to pay $45,000 for selling food passed the used by date.
A barbecue business was also convicted of Food Act breaches and fined $30,000 in January.
