Ludhiana based surgeon Dr Ranbir Singh, consultant surgeon and specialist in Laparoscopic surgery and fellow from Washington university St Louis, USA achieved a distinguished feat of operating gall bladder surgery (Laparoscopic cholecystectomy) for the LEFT SIDED GALL BLADDER.

Patient – Hubraji 62yrs/female from Sultanpur UP, now staying in Mandi Gobindgarh presented with dyspepsia , gas formation and pain on the left side of the abdomen over the stomach. As it was unusual to think about gall stone problem, because gall bladder normally is placed on the right side. It was diagnosed on Ultrasonography, which showed the gall bladder is inflamed and full of stones is lying on the left side of the abdomen. The stomach and heart were lying on the opposite sides and other organs were also reversed.

Dr Singh elaborated that this condition is known as “SITUS INVERSUS” and is a congenital autosomal recessive genetic disorder which occurs in one : 10-20000 live births and in the world literature 60-70 cases of laparoscopic cholecystectomy have been operated so far. Dr Kuldip Singh, Team leader and one of the first to start laparoscopic surgery in northern India and has experienced third such case in his 27 yrs of laparoscopic career, says that very few surgeons have experienced this condition more than once. Regarding the technical difficulties, he points out that as the organs are lying in opposite directions and laparoscopic surgery is performed by watching the TV screen, the hand eye coordination, orientation to the structures and movements of the instruments becomes very difficult for right handed surgeon.

Dr RP Singh Chief Cardiologist of Pancham Hospital , who evaluated her cardiac status and found her heart functioning at a rate of 38% only, which adds to the risk to undergo Laparoscopic surgery and that too in a difficult condition of “Situs inversus” . Dr Kuldip Singh further explained that he was well aware of the difficulties by his past experience to help his team in operating with much ease in spite of comorbid conditions without any difficulty and standardizing the operative technique to tackle such cases . Patient was discharged on the second day and in a satisfactory condition.
