This peer-reviewed article along with its instructional video describes intraoperative visual axis identification using patient fixation, a new surgical procedure.

In this paper, the study’s author, Dr. Vance Thompson (Vance Thompson Vision, Sioux Falls, South Dakota) describes a procedure of performing intraoperative Zepto-guided™ visual axis centration during cataract surgery. This procedure translates the patient’s visual axis into a surgical reference landmark that guides the surgeon’s placement of the capsulotomy and the IOL. This intraoperative axis identification and centration method is based on the use of patient fixation, co-axial microscope optics and Purkinje images as originally described by Daniel Chang M.D. and George Waring IV M.D.
“By using Zepto-guided™ visual axis identification, I have a fixed frame of reference for the patient’s visual axis during surgery” explains Dr. Thompson. “This helps eliminate guesswork on where to position the capsulotomy and the Zepto-guided™ capsulotomy almost always ends up being where the IOL naturally positions itself within the capsular bag.” Dr. Thompson adds “I now consistently obtain complete capsular overlap in all cases, which we all know provides greater stability against lens tilt and decentration.” “Additionally, positioning on the visual axis is important for best optical performance from premium IOLs and together with less lens tilt may help minimize dysphotopsias.” “The paper was submitted last October and was based on my first 86 consecutive cases. “I now routinely use this Zepto® guidance and have performed this procedure now in over 1,000 cases.”
