Chelsea have endured a miserable campaign and saw their title defence fold spectacularly as Manchester City romped to the Premier League.
The Blues are expected to undergo a massive overhaul of their squad in the summer and recent reports have suggested the west London outfit could release as many as NINE players.
Real Madrid have been courting Hazard for a while and Zinedine Zidane is supposedly looking at the Belgium international as a potential replacement for Gareth Bale, who has been plagued with injuries over the past few seasons.
Roman Abramovich, meanwhile, appears to be ready to replace Conte and Napoli’s Maurizio Sarri and Luis Enrique are at the top of his wishlist.

To sack Conte could cost the Blues as much as £9million.
And according to the Daily Telegraph, Hazard’s future could affect which manager Abramovich decides to appoint.
As of yet, Hazard has yet to commit to a contract extension even though Chelsea are prepared to offer him a new £300,000-a-week deal.
It is said that hiring Sarri or Enrique could persuade Hazard to stay at Stamford Bridge as they are both more attack-minded in their philosophies.
As far as Real are concerned, they are yet to make a formal approach and they are still weighing up their options.
Los Blancos are currently exploring a potential deal for Paris Saint-Germain star Neymar who is currently sidelined with injury.
