The Oklahoma City Police Department has issued a Silver Alert for a missing 66-year-old woman, Monday.

Paula Juarez, was last seen around 4 p.m. at an Oklahoma City apartment in the 5900 block of S. May, Saturday, April 28.

Juarez left a suicide note before turning off her cellphone, according to report.

Juarez has very short gray hair and is driving a newer model Dodge pickup truck.

Officials said Juarez has a proven medical or physical disability and is in imminent danger of serious bodily injury or death.
