All eyes are on the outpatient, primary care setting where clinicians and staff are developing new, team-based, whole-person approaches to physical, social, and behavioral health for patients, in collaboration with community services and supports. With a changing health care landscape and a continuing shift to value-based care, Summit speakers and sessions will highlight efforts across the country to improve care for individuals and populations; focus on health, well-being, and equity across diverse communities; and ensure health professionals and staff have the supports and resources they need to achieve these broadening goals.
