SYCAMORE – A new health partnership, Together for a Healthier DeKalb County, is asking all county residents to help identify pressing community health needs through an online survey.
The Northwestern Medicine Kishwaukee and Valley West hospitals, DeKalb County Health Department and community stakeholders are collaborating on a project to assess the health needs of the residents of DeKalb County.
The survey is conducted once every three years by Northwestern Medicine KishHealth System and Valley West Hospitals and once every five years by the county health department. The information will be combined with federal, state and local health data to help determine health priorities over the next few years.
The survey is available through the health department's website, dekalbcounty.org/health. It is available in English and Spanish and will take about 15 to 20 minutes to complete. Printed copies are available from the DeKalb County Health Department, from 8 a.m. to 4:30 p.m. Monday through Friday, at 2550 N. Annie Glidden Road, DeKalb.
